About Crossroads of Hope, Inc.
Crossroads of Hope, Inc. has provided Supports for Community Living (SCL) services since March 2007. Over the years, our agency has expanded the range of services we offer, including those for participants approved under the Michelle P. Waiver (MPW). Crossroads of Hope, Inc. is committed to providing high-quality care and support to individuals with intellectual and developmental disabilities, and we are dedicated to helping them live a more fulfilling life in their communities.
Crossroads of Hope, Inc. has provided Supports for Community Living (SCL) services since March 2007. Over the years, our agency has expanded the range of services we offer, including those for participants approved under the Michelle P. Waiver (MPW). Crossroads of Hope, Inc. is committed to providing high-quality care and support to individuals with intellectual and developmental disabilities, and we are dedicated to helping them live a more fulfilling life in their communities.
Services and Supports we offer:
Residential (SCL): Our Residential Supports program provides individuals with a safe and comfortable home environment where they can receive 24-hour supervision and training in activities of daily living. Whether it's learning how to cook, managing money, or participating in social and leisure activities, Residential Supports are tailored to meet the unique needs and wants of each individual. With the option of either a staffed residence or family home provider, the program allows individuals to live in small, individualized settings with up to three recipients of waiver services.
Day Training (SCL and MPW): Day Training is designed to help people lead more independent lives. Day Training helps support recipients to participate in daily meaningful routines in the community, and it stresses training in the activities of daily living, self-advocacy, adaptive and social skills. On-site services should lead to the acquisition of skills and abilities for work and/or community participation. Off-site services are provided in a variety of community settings, promoting inclusion and natural support.
Positive Behavior Support Services (SCL and MPW): We provide Positive Behavior Support Services, understanding the unique challenges some participants may encounter. Through comprehensive assessment and strategic planning, our Positive Behavior Supports focus on replacing challenging behaviors with more constructive alternatives. By empowering participants to adopt positive changes in their lives and environments, we enable them to embrace life to the fullest.
Supported Employment (SCL and MPW): Supported Employment enables participants to engage in paid work in an integrated community setting with competitive wages and benefits. Our team assists with all phases of obtaining employment from assisting with making referrals to the Office of Vocational Rehabilitation (OVR), Person-Centered Employment Planning, Job Development, Job Acquisition, and Long Term Follow-Up. Our team continues long term support and follow-up to help individuals maintain their jobs and continue to be successful in the workplace, even after the Supported Employment Specialist is no longer needed on a regular basis.
Community Access (SCL): We also offer Community Access, a service that helps individuals connect and become involved with clubs, associations, and other groups in the community. This service is focused on promoting independence, developing social networks, and fostering friendships and relationships for the participants in the community.
